A Leader in Natural Facial Rejuvenation

Dr. Sadati has dedicated his career to facial rejuvenation that looks beautifully natural and never overdone. He’s best known for developing the Preservation Deep Plane Facelift — a sophisticated technique that lifts and repositions the deeper layers of the face, rather than simply pulling the skin tight. By preserving the natural blood supply and minimizing unnecessary tissue disruption, this method helps reduce swelling, shortens downtime, and preserves the soft, expressive look that patients value most. He also performs his facelift surgeries using local anesthesia combined with twilight sedation, a safer alternative to general anesthesia that reduces risk and allows for a gentler, more seamless recovery experience. His technique has been published in the prestigious Plastic & Reconstructive Surgery Journal, and has been presented at national and international conferences. Today, it’s embraced by surgeons worldwide for its ability to deliver results that are both natural-looking and long-lasting.

The Evolution of Dr. Sadati’s Facelift Technique:From Mini Lifts to the Preservation Deep Plane Facelift

Facelift surgery has come a long way in the past 30 years. What started with minimal skin tightening has turned into advanced facial rejuvenating techniques targeting the deep facial planes for a longer-lasting, more naturally appearing outcome. One surgeon who has played a leading role in developing the field is Dr. Kevin Sadati, a facial plastic surgeon who is board certified, based in Newport Beach, California.

It began with the mini facelift—a less drastic facelift operation designed for fixing early signs of aging in the lower face and the region near the jawline. While good for mild sagging, results were generally fleeting. For his patients to achieve improved, longer-lasting results, Dr. Sadati refined the Double C SMAS Plication Facelift Technique and perfected it in over 1,500 cases, publishing his outcomes in the American Journal of Cosmetic Surgery. It lifted the SMAS layer with a double “C” pattern suture, improving the look of the jawline, as well as the neck. It delivered stronger facelift results than earlier strategies—but still had its weaknesses, especially in the midface region, also in the nasolabial folds.

To enhance support and midface correction, he created the Triple C SMAS Plication Technique, published in the Plastic & Reconstructive Surgery Journal. This added a third vector of lift, leading to better contouring. However, like many SMAS facelift methods, it was still based on folding tissue rather than fully mobilizing it, which restricted how much correction was possible for deeper facial aging.

Dr. Sadati then proceeded with the Extended Deep Plane Facelift, where the tissues are lifted below the SMAS layer with release of significant retaining ligaments. It allowed for fuller tissue relocation and greater midface rejuvenation. However, the technique entailed sacrifices: increased swelling, longer recovery period, yet high incidence of seromas/hematomas formation.

To solve these issues, Dr. Sadati developed the Preservation Deep Plane Facelift—a cutting-edge technique that merges the effectiveness of a deep plane facelift with a gentler, more anatomically respectful approach. This technique, blending aspects of the Extended Deep-Plane and High Lateral SMAS Facelifts, aims to optimize outcomes while minimizing risks and recovery duration. The term Preservation facelift refers to limited skin dissection in the face and neck areas to reduce complications while achieving extended deep plane facelift results. It also aims to preserve anatomical structures as much as possible. The Preservation Facelift also incorporates a rotating pedicled SMAS flap along the jawline. This maneuver aims to sharpen the jawline. Better neck contouring is achieved through minimal skin elevation but working on the deep neck muscles called platysma creating a hammock with unique quilting sutures.

This modern technique is ideal for a wide range of ages, since it is performed under twilight sedation like colonoscopy IV sedation, avoiding the need for general anesthesia, it significantly lowers the risk of complications. The Preservation Deep Plane Facelift reflects Dr. Sadati’s commitment to pushing the boundaries of facial plastic surgery, offering patients from all corners of the world a better facial rejuvenation technique.

Dr. Sadati has authored numerous original facial rejuvenation procedures, including his revolutionary Preservation Deep Plane Facelift, in the highly respected Plastic & Reconstructive Surgery Journal. His innovative technique has been well received throughout the aesthetic surgery community. He has presented his methods and results in countless national and international medical society meetings, where he is often a guest speaker due to his expertise in facial plastic surgery. These offerings are a testament not only to his leadership in developing surgical procedures, but also his dedication to informing fellow surgeons throughout the globe.
